Lost Policy Rate
If an insured applies for a replacement Policy, rather than a Company certified copy of an existing policy, the charge shall be $50.00. The insured must submit a written request for policy. The new policy will bear the following endorsement on its face:
"This Policy is issued in lieu of lost Policy number _____________ which is hereby canceled."
